Liver transplantation combined with pancreatectomy for the treatment of metastatic pancreatoblastoma-three cases

Objective: To investigate the effectiveness of combined liver transplantation and pancreatectomy in the treatment of pancreatoblastoma with liver metastasis.
Method: The clinical data of 3 children with pancreatoblastoma combined with liver metastasis in our hospital from 2022 to 2023 were retrospectively analyzed.
Results: All 3 children were pathologically diagnosed as pancreatoblastoma preoperatively, one tumor located in the middle segment of the pancreas, and the middle segment of the pancreas was resected pancreatic tail-jejunum anastomosis was performed preoperatively, the other two were located in the head of the pancreas, 3 patients were combined with extensive hepatic metastasis and hilar lymph node metastasis by CT and PET-CT and other metastases such as lung were excluded. Two cases were performed with liver transplantation combined with pancreatoduodenectomy, one patient was performed with liver transplant combined with lymph node dissection in the abdominal cavity. Postoperative pathological examination confirmed pancreatoblastoma. All 3 children were treated with chemotherapy after operation. Two children not have recurrence after operation, and one child had recurrence after operation, and severe pneumonia occurred during chemotherapy, leading to death.
Conclusion: Pancreatoblastoma in children can be accompanied by liver metastases, and liver transplantation combined with pancreatectomy is the treatment method for liver metastases, and attention should be paid to complete resection of lesions during surgery.
